It gives the address as Holly Cottage and says that John served for 2 years. It was signed on 21st September 1919 by Mr Joseph Davies. UK Soldiers who died in The Great War 1914 -19 accessible on www.ancestry.co.uk says that he was born in Halkyn and enlisted in Wigan.
